Supporting your family, together

You give so much of yourself — at work, at home, and in caring for your loved one. We’re here to provide the support you deserve, so you can find balance beyond caregiving and know your loved one is in good hands.

For parents interested in becoming paid caregivers, Arizona’s Parents as Paid Caregivers (PPCG) program offers an opportunity to provide care while receiving compensation. For those enrolled in this program, we also try to work with your insurance to work along side you, if requested, providing a weekly caregiver support to give you a break (Respite). • Locally Based and Reliable: Our caregivers live right here in the community — the same neighborhoods, schools, and stores as the families we serve. That means dependable care from people who understand and care about your family’s daily life. We make every effort to send the same caregiver, everytime
  • Supported and Supervised: We don’t just hire and send caregivers — we stay involved. Our team receives ongoing training and supervision to make sure every visit reflects our commitment to excellence and your loved one’s individualized plan

What services can be provided as a Parent Provider?

Parent Providers can provide Habilitation and Attendant Care services to their own child through DDD. We guide families through each step — from training and documentation to meeting all DDD and AHCCCS requirements — so you can focus on caring for your loved one with confidence and support.

Are your services covered by insurance?

We are currently working towards accepting DDD, TriWest, and Banner through AHCCCS. We also accept private pay. Call us for the latest status 602-641-4814

What areas in Arizona do you serve?

We currently serve the West Valley of Phoenix, so everything that is to the west of I-17 until the 303. This includes Surprise, Glendale, Peoria, Goodyear, and Litchfield Park. We also have an expansive network of other providers for private pay inquiries needing services in Tucson or the East Valley

How quickly can services with our new caregiver start once I'm approved?

We maintain an active roster of qualified, trained, and certified caregivers. Once your case manager approves you for service with us, we can usually start within 4 weeks or less

What qualifications do your caregivers have?

Our caregivers are all experience and hold certifications in CPR, Article 9, and are fingerprinted. They also receive ongoing training to stay current with best practices and advancements. In addition, we have an experienced registered nurse and medical assistant professor on staff who providers on-going training

What does a typical day/visit look like? How many hours per week can I receive?

When you start working with us, while we're waiting to assign your caregiver, we'll go through a simple orientation with you. We put together a care plan based on your loved ones care team, which consists of someone close to them (perhaps that's you!), a member of the Convini Leadership Team, and your Case Manager.

The care plan will determine that types of activities that will occur throughout the week. In terms of hours per week, it varies, we're seeing anywhere between 10-25 hours per week on average. Sometimes a little less, sometimes a little more
